Overview

sCD44v6CAR is an experimental second-generation chimeric antigen receptor (CAR) T-cell therapy designed to target the CD44v6 splice variant, which is highly expressed in hematological malignancies such as acute myeloid leukemia (AML) and multiple myeloma (MM). To address the challenge of "on-target, off-tumor" toxicity—as CD44v6 is also expressed on healthy tissues like keratinocytes—this construct is engineered to secrete a soluble, protease-susceptible version of the anti-CD44v6 single-chain variable fragment (scFv). This soluble scFv contains an engineered matrix metalloproteinase-2 (MMP-2) cleavage site within its linker. In healthy tissues, the soluble scFv binds and masks CD44v6 antigens with high affinity, effectively blocking CAR T-cell recognition and activation. In the tumor microenvironment, where MMP-2 is significantly overexpressed, the soluble scFv is cleaved, allowing the CAR T cells to bind the membrane-bound CD44v6 on malignant cells and exert their cytotoxic effects.
