SCH-486757 is a selective, orally active small molecule agonist of the nociceptin/orphanin FQ peptide (NOP) receptor. It was developed as an investigational antitussive agent for the treatment of cough. Unlike classical opioid antitussives, SCH-486757 demonstrates high selectivity for the NOP receptor over traditional opioid receptors and does not display potential for abuse or dependency in preclinical models. The drug showed efficacy in suppressing cough in animal models comparable to established antitussives such as codeine and dextromethorphan. Despite promising preclinical results and advancement into phase II clinical trials for chronic cough, development was discontinued.
