scp776 is a first-in-class targeted insulin-like growth factor 1 (IGF-1) fusion protein developed by Silver Creek Pharmaceuticals. It is designed to target damaged tissue and promote apoptosis escape, thereby mitigating cell loss and infarct expansion in acute ischemic injuries such as acute ischemic stroke (AIS) and acute myocardial infarction (AMI). The drug leverages IGF-1’s pro-survival signaling but overcomes the limitations of native IGF-1 by fusing it with Annexin A5, which enables selective targeting of apoptotic cells through interaction with phosphatidylserine exposed on their surface. This results in more than 100-fold higher potency on injured cells compared to healthy ones. scp776 is administered intravenously and has demonstrated good tolerability in phase 1 studies, with adaptive blood glucose management protocols implemented due to its potential for hypoglycemia. It is currently in phase 2 clinical trials for acute ischemic stroke and ST-elevation myocardial infarction, with additional development for brain injury and chemical-induced lung injury.
