Overview

sdAb5-based BCMA CAR-T cells are an experimental second-generation chimeric antigen receptor (CAR) T-cell therapy targeting B-cell maturation antigen (BCMA) for the treatment of multiple myeloma. Developed by Cima Universidad de Navarra and Nanogrow Biotech, the therapy utilizes a single-domain antibody (sdAb) derived from immunized llamas (clone sdAb5) as the antigen-recognition moiety, coupled with a 4-1BB costimulatory domain. In preclinical xenograft models, sdAb5-based CAR-T cells demonstrated superior antitumoral efficacy and persistence compared to approved BCMA CAR-T therapies like idecabtagene vicleucel and ciltacabtagene autoleucel. The therapy is characterized by a unique Th2 polarization that transitions into CD8+ effector and memory-like cells, enabling robust re-expansion and sustained protection upon tumor rechallenge.
