Seladelpar is a small molecule drug that acts as a potent and selective agonist of the peroxisome proliferator-activated receptor delta (PPARδ). It is indicated for the treatment of primary biliary cholangitis (PBC), a chronic liver disease characterized by progressive destruction of bile ducts. Seladelpar can be used in combination with ursodeoxycholic acid (UDCA) in adults who have an inadequate response to UDCA or as monotherapy in those unable to tolerate UDCA[1][2][3][5][8]. The drug works by activating PPARδ, which leads to reduced bile acid synthesis through fibroblast growth factor 21 (FGF21)-dependent downregulation of cholesterol 7 alpha-hydroxylase-1 (CYP7A1), the key enzyme responsible for converting cholesterol into bile acids[5][6][8]. This mechanism helps decrease liver inflammation and fibrosis associated with PBC. Seladelpar received FDA accelerated approval under the brand name Livdelzi on August 14, 2024[3].
