Selegiline + alpha-tocopherol is a combination therapy consisting of two agents: - Selegiline, a selective monoamine oxidase B (MAO-B) inhibitor that increases brain catecholamine levels by inhibiting their breakdown. It is primarily used in the management of Parkinson's disease and has been investigated for its neuroprotective effects in Alzheimer's disease. - Alpha-tocopherol (vitamin E), a fat-soluble antioxidant that protects cell membranes from oxidative damage. This combination was studied for its potential to slow the progression of moderately severe Alzheimer's disease. Clinical trials demonstrated that both agents individually delayed clinical deterioration—measured as time to death, institutionalization, loss of basic activities of daily living, or severe dementia—compared to placebo. However, the combination did not provide additional benefit over either agent alone and was associated with an increased risk of falls and syncope[1][2][5]. The mechanism involves MAO-B inhibition (selegiline) leading to increased dopamine availability and antioxidant protection (alpha-tocopherol)[1][5].
