XAB is a combination regimen consisting of selinexor, azacitidine, and venetoclax, specifically designed as a chemotherapy-sparing or low-dose chemotherapy approach for treatment-naïve patients with acute myeloid leukemia (AML) who are unfit for intensive induction therapy. Selinexor is a first-in-class oral selective inhibitor of nuclear export (SINE) that targets Exportin-1 (XPO1), thereby blocking the nuclear export of tumor suppressor proteins and restoring their natural antitumor activity. Azacitidine acts as a hypomethylating agent by inhibiting DNA methyltransferase, while venetoclax is a selective B-cell lymphoma 2 (BCL-2) inhibitor that promotes apoptosis in leukemia cells. The regimen is being evaluated for its ability to achieve rapid blast cell reduction and dynamic clearance with a manageable toxicity profile in elderly or frail patient populations.
