This drug is an investigational combination therapy consisting of **selinexor**, **dexamethasone**, **pomalidomide**, and **daratumumab**, used primarily in the treatment of **relapsed or refractory multiple myeloma**. - **Selinexor** is a first-in-class, oral selective inhibitor of nuclear export (SINE), targeting exportin 1 (XPO1), which leads to the retention of tumor suppressor proteins in the nucleus and induction of apoptosis in malignant cells. - **Dexamethasone** is a synthetic glucocorticoid used to reduce inflammation and induce apoptosis in multiple myeloma cells. - **Pomalidomide** is an immunomodulatory agent (IMiD) that exerts antimyeloma activity through direct cytotoxic effects, modulation of the tumor microenvironment, and enhancement of immune response. - **Daratumumab** is a humanized monoclonal antibody targeting the CD38 glycoprotein on myeloma cells, inducing cell death through antibody-dependent cell-mediated cytotoxicity and complement-dependent cytotoxicity. Preclinical and early-phase clinical studies indicate these agents, when combined, may provide a **synergistic anti-myeloma effect** and restore responsiveness in patients refractory to established regimens containing IMiDs, proteasome inhibitors, or anti-CD38 monoclonal antibodies[1][2][3][4][5][6][7][8].
