Selinexor + FOLFIRI is an experimental combination therapy used in clinical trials for relapsed or metastatic, refractory solid tumors. Selinexor is an oral selective inhibitor of nuclear export (SINE) that inhibits exportin 1 (XPO1), disrupting the transport of tumor suppressor proteins out of the nucleus, thereby enhancing their tumor-suppressive functions and sensitizing cancer cells to DNA damage. FOLFIRI is a standard chemotherapy regimen comprising irinotecan (a topoisomerase I inhibitor), fluorouracil (an antimetabolite disrupting DNA synthesis), and folinic acid (leucovorin, which enhances the effect of fluorouracil). Together, the drugs aim to exploit impairments in DNA repair and cell cycle regulation to increase cancer cell death. The combination has demonstrated some disease stabilization in heavily pre-treated patients, but clinical activity has been limited by toxicity, particularly myelosuppression and gastrointestinal adverse events[1][7][5].
