Overview

A combination chemotherapy regimen consisting of **selinexor**, **ifosfamide**, **carboplatin**, and **etoposide**. Selinexor is a selective inhibitor of nuclear export (SINE) that blocks exportin-1 (XPO1), leading to nuclear retention and activation of tumor suppressor proteins such as p53, p73, and FOXO, resulting in apoptosis in cancer cells. Ifosfamide is an alkylating agent that induces DNA cross-linking and cell death. Carboplatin is a platinum-based compound that causes DNA damage, and etoposide is a topoisomerase II inhibitor that also induces DNA strand breaks. This combination has been investigated primarily in relapsed or refractory peripheral T-cell lymphoma (TCL) and natural-killer/T-cell lymphoma (NKTL), where it has shown high response rates (overall and complete response rates up to 91% and 82%, respectively), with significant adverse hematologic toxicity. The combination is investigational and not approved for routine clinical use[3][5][7].
