**Selinexor + lenalidomide + tafasitamab** is an investigational combination therapy that includes three distinct agents with complementary mechanisms of action, primarily evaluated for treatment of relapsed or refractory diffuse large B-cell lymphoma (DLBCL), especially in patients who may not be candidates for autologous stem cell transplantation. \n- **Selinexor** is a small molecule selective inhibitor of nuclear export (SINE) that blocks exportin 1 (XPO1), leading to accumulation of tumor suppressor proteins inside the nucleus and ultimately apoptotic cell death of malignant cells. \n- **Lenalidomide** is an immunomodulatory drug that enhances T-cell and natural killer (NK) cell function, disrupts tumor-promoting microenvironments, and induces direct antineoplastic effects. \n- **Tafasitamab** is a humanized anti-CD19 monoclonal antibody engineered to enhance both antibody-dependent cellular cytotoxicity (ADCC) and antibody-dependent cellular phagocytosis (ADCP), directly targeting B cells expressing CD19 and facilitating their immune-mediated destruction. \nThis triplet is under investigation but not approved as a fixed commercial combination; however, each component is approved or in use for certain lymphoid malignancies.
