SEMA4A CAR T cells are an investigational chimeric antigen receptor (CAR) T-cell therapy designed to target Semaphorin 4A (SEMA4A), a protein highly expressed in multiple myeloma (MM) cells, including those with p53 mutations and those that have relapsed after BCMA-targeted CAR T-cell therapy. Developed by researchers at the H. Lee Moffitt Cancer Center and Research Institute, these CAR T cells aim to address the challenge of BCMA antigen loss or low density, which is a common mechanism of relapse in MM patients treated with BCMA-directed therapies. Preclinical studies demonstrate that SEMA4A is expressed at significantly higher levels than BCMA in relapsed patients and that SEMA4A-directed CAR T cells can effectively eliminate MM cells in vitro and in vivo without significant toxicity to normal tissues or hematopoietic progenitors.
