Overview

Semzuvolimab (formerly known as UB-421) is an investigational monoclonal antibody developed for the treatment of HIV infection. It is a humanized IgG1 antibody that targets domain 1 of the CD4 receptor on CD4+ T cells. By competitively binding to this site with high affinity—much stronger than HIV itself—it blocks the initial attachment and entry of HIV into host immune cells. This mechanism prevents both cell-free and cell-to-cell transmission of HIV and is effective against multiple subtypes and drug-resistant strains. Unlike some other antibodies targeting viral proteins directly (which can lead to resistance due to rapid viral mutation), semzuvolimab targets a stable human protein used by all HIV variants for entry, reducing the risk of resistance development. Clinical studies have shown that it can suppress viral load in people with multidrug-resistant (MDR) HIV who have limited or no remaining treatment options[1][5][6][7].
