Overview

Sequential therapy for Helicobacter pylori infection is a 10-day treatment regimen specifically evaluated by Changi General Hospital in Singapore. The regimen is administered in two distinct phases: an initial 5-day induction phase consisting of a proton pump inhibitor (PPI) and amoxicillin, followed by a 5-day eradication phase comprising a PPI, clarithromycin, and metronidazole. The rationale behind this sequential approach is that amoxicillin weakens the bacterial cell wall during the first phase, potentially preventing the development of drug efflux channels that could otherwise inhibit clarithromycin from binding to ribosomes, thereby enhancing the efficacy of the second-phase antibiotics. Although studied in a randomized clinical trial against standard triple and concomitant therapies, sequential therapy did not demonstrate superiority for first-line H. pylori eradication in the Singaporean population.
