Overview

SER-287 + vancomycin is a combination therapy consisting of oral SER-287, a spore-based microbiome therapeutic derived from Firmicutes bacteria, and oral vancomycin, an antibiotic. This regimen is being investigated for the treatment of mild-to-moderate ulcerative colitis (UC). Vancomycin is used as a short preconditioning course to reduce native gut microbiota diversity and open ecological niches in the gastrointestinal tract. This facilitates robust engraftment of the beneficial bacterial spores delivered by SER-287. The mechanism involves modulation of the gut microbiome composition—specifically increasing spore-forming Firmicutes associated with colonic homeostasis and reducing pro-inflammatory species such as Enterobacteriaceae—leading to changes in microbe-associated metabolites that may improve epithelial barrier function and immune signaling. Clinical studies have shown that this combination can induce clinical remission in UC patients; however, efficacy results have been mixed across different trial phases[1][2][5][6][7].
