Overview

SerpinPC is a recombinant, engineered serine protease inhibitor (serpin), specifically a mutated alpha-1-antitrypsin, designed to treat hemophilia. It works by selectively inhibiting activated protein C (APC), which normally downregulates thrombin generation and limits blood clotting. By reducing APC activity, SerpinPC increases thrombin production at sites of tissue damage and prolongs the activity of prothrombinase, thereby promoting effective blood clot formation in patients with hemophilia A or B regardless of disease severity or inhibitor status. The drug was developed as a subcutaneously administered therapy and demonstrated significant reductions in annualized bleeding rates in clinical trials[1][2][3][4][5][6][7]. Development of SerpinPC for hemophilia was discontinued by Centessa Pharmaceuticals in November 2024 despite promising clinical data[4]. The drug received Orphan Drug Designation and Fast Track designation from the FDA for hemophilia B[3][7].
