This combination therapy regimen is being evaluated in the UNICORN study (NCT05423392), a Phase II clinical trial led by Hubei Cancer Hospital for elderly patients (≥65 years) with stage IV metastatic non-small cell lung cancer (NSCLC). The regimen consists of the PD-1 inhibitor serplulimab combined with a single-agent chemotherapy, which is selected based on tumor histology: paclitaxel or nab-paclitaxel for squamous NSCLC, and pemetrexed for lung adenocarcinoma. Serplulimab is a humanized monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor, blocking its interaction with PD-L1 and PD-L2 to restore T-cell mediated anti-tumor immune responses. This treatment strategy is specifically designed for elderly patients who may have reduced tolerance for standard platinum-based doublet chemotherapy regimens, aiming to maintain efficacy while improving quality of life and reducing adverse reactions.
