Overview

This refers to a specific inhalational anesthesia regimen utilized in a clinical study conducted by Yonsei University for patients undergoing transsphenoidal surgery for pituitary tumors. The protocol involves induction with the barbiturate pentothal sodium (thiopental) at 4-6 mg/kg and the synthetic opioid remifentanil at 1 μg/kg, followed by maintenance with the volatile anesthetic sevoflurane (0.8 to 1 MAC) and a continuous infusion of remifentanil (0.1-0.3 μg/kg/min). Sevoflurane provides general anesthesia through the modulation of GABA-A and NMDA receptors, while remifentanil offers potent, short-acting analgesia via mu-opioid receptor agonism. Thiopental is used for rapid-onset induction. The clinical trial (NCT02443311) specifically evaluates the quality of recovery (QoR-40) of this inhalational approach compared to total intravenous anesthesia (TIVA) using propofol and remifentanil.
