SGN-ALPV is a novel, investigational antibody-drug conjugate (ADC) composed of a humanized monoclonal antibody targeting placental alkaline phosphatase (ALPP) and ALPP-like 2 (ALPPL2), conjugated via a protease-cleavable linker to the microtubule-disrupting agent monomethyl auristatin E (MMAE). The drug binds to ALPP or ALPPL2 on the surface of tumor cells, is internalized, and releases MMAE in the cytoplasm after lysosomal cleavage. MMAE then binds tubulin, inhibiting microtubule polymerization, leading to cell cycle arrest and apoptosis. Additional mechanisms include immunogenic cell death and bystander killing of neighboring cells. SGN-ALPV was developed for cancers with high expression of ALPP/ALPPL2 such as ovarian, endometrial, gastric, testicular germ cell tumors, non-small cell lung cancer (NSCLC), cervical cancer, and other advanced solid tumors[1][2][6].
