SGN-CD123A is an investigational antibody-drug conjugate (ADC) designed for the treatment of acute myeloid leukemia (AML). It consists of a humanized monoclonal antibody targeting CD123 (the interleukin-3 receptor alpha chain), which is highly expressed on AML blasts and leukemic stem cells, but less so on normal hematopoietic cells. The antibody is site-specifically conjugated via engineered cysteines to a potent DNA-binding pyrrolobenzodiazepine (PBD) dimer cytotoxic payload using a protease-cleavable linker. Upon binding to CD123-expressing cells, the ADC is internalized, releasing the PBD dimer inside lysosomes, leading to DNA crosslinking, activation of DNA damage response pathways, cell-cycle arrest, and apoptosis in AML cells. Preclinical studies demonstrated potent antileukemic activity in vitro and in vivo models resistant to conventional chemotherapy. SGN-CD123A was developed by Seattle Genetics and entered phase 1 clinical trials for relapsed or refractory AML[2][3][5][8].
