Shenfu Decoction is a traditional Chinese medicine formula composed primarily of Panax ginseng (Ren Shen) and processed lateral root of Aconitum carmichaeli (Zhi Fu Zi). It has been used for centuries to treat conditions associated with Yang collapse in traditional Chinese medicine. In modern clinical research and practice, it is most commonly used as an adjunctive therapy for heart failure and cardiovascular diseases. The main active components are ginsenosides from ginseng and aconitum alkaloids from aconite. Mechanistically, these compounds enhance cardiac contractility, increase coronary blood flow, improve myocardial metabolism under ischemic conditions, scavenge free radicals, protect myocardial ultrastructure, modulate inflammatory mediators (such as TNF-α), regulate neurohormonal factors (including angiotensin converting enzyme [ACE], renin [REN], aldosterone [ALD], atrial natriuretic peptide [ANP]), inhibit apoptosis pathways (e.g., Bcl-2/caspase-3), improve neutrophil chemotaxis via modulation of P2X1 receptor expression and chemokine receptors CXCR1/CXCR2/FPR1. It is not a single-molecule pharmaceutical but a multi-component herbal decoction[1][3][4][6].
