Shin-yi-san is a classic Traditional Chinese Medicine (TCM) herbal formula primarily utilized for the treatment of nasal and respiratory conditions, most notably allergic rhinitis, sinusitis, and nasal congestion. The formula's primary active component is derived from *Magnolia biondii* (Xinyi), which is typically combined with other medicinal herbs such as *Angelica dahurica*, *Mentha haplocalyx*, and *Glycyrrhiza uralensis*. Its therapeutic profile is characterized by multi-target immunomodulatory and anti-inflammatory activities. Research indicates that Shin-yi-san reduces serum IgE levels, suppresses the production of Th2-associated cytokines like Interleukin-5 (IL-5) and Th1 cytokines like Interferon gamma (IFN-gamma), while simultaneously enhancing the production of the anti-inflammatory cytokine Interleukin-10 (IL-10). It also demonstrates the ability to suppress cyclooxygenase-2 (COX-2) mRNA expression in neutrophils. In clinical practice, particularly in Taiwan, it is frequently co-prescribed with other formulas such as Xiao-qing-long-tang to manage perennial allergic rhinitis.
