Overview

Short-chain fructooligosaccharide (scFOS) is a prebiotic dietary fiber being evaluated by the Mayo Clinic for the prevention of pouchitis in patients with ulcerative colitis following ileal pouch-anal anastomosis (IPAA). scFOS consists of short chains of fructose molecules that are resistant to digestion in the human upper gastrointestinal tract. Upon reaching the colon, they are selectively fermented by beneficial bacteria, particularly *Bifidobacterium* species. This fermentation process promotes the production of short-chain fatty acids (SCFAs) and modulates the gut microbiome, which is hypothesized to reduce the inflammatory processes leading to pouchitis. The Mayo Clinic has conducted double-blind, placebo-controlled clinical trials to assess whether daily supplementation with scFOS chews can prevent the development of this inflammatory condition after surgical ileostomy closure.
