Shosaikotokakikyosekko is a traditional Japanese Kampo medicine (TJ-109) formulated as an extract of nine herbal and mineral ingredients: Bupleurum root, Scutellaria root, Ginseng, Pinellia tuber, Ginger rhizome, Jujube fruit, Glycyrrhiza root, Platycodon root, and Gypsum. It is a modification of the classic Shosaikoto (Minor Bupleurum Decoction) with the addition of Platycodon and Gypsum to enhance its therapeutic effects on throat inflammation and febrile conditions. Traditionally indicated for tonsillitis and peritonsillitis, it gained significant attention in Japan during the COVID-19 pandemic for the symptomatic treatment of coronavirus disease 2019, particularly for alleviating sore throat and fever. The medicine exerts its effects through a multi-component mechanism that includes the inhibition of pro-inflammatory cytokines such as IL-6 and TNF-alpha, as well as the modulation of the immune response. Clinical trials in Japan have evaluated its efficacy in reducing the duration of symptoms and preventing disease progression in patients with mild-to-moderate COVID-19.
