Overview

SHR2554 + aza + overlapped mBUCY is an experimental conditioning regimen being evaluated for patients with high-risk or relapsed/refractory acute leukemia (including acute myeloid leukemia and acute lymphoblastic leukemia) and myelodysplastic neoplasms (MDS) undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT). The regimen integrates **SHR2554**, a potent and selective small-molecule inhibitor of Enhancer of Zeste Homolog 2 (EZH2), and the hypomethylating agent **azacitidine** into a modified busulfan and cyclophosphamide (**mBUCY**) conditioning backbone. The mBUCY component typically consists of semustine, cytarabine, busulfan, and cyclophosphamide. This combination is designed as an overlapped sequential treatment, where the epigenetic modulators (SHR2554 and azacitidine) are administered alongside intensive chemotherapy to potentially sensitize malignant cells and reduce the risk of post-transplant relapse. The regimen is currently being studied in a multicenter randomized controlled trial led by The First Affiliated Hospital of Soochow University.
