Overview

Si Ni San is a classic traditional Chinese medicine (TCM) formula composed of four herbs: Radix Bupleuri (Bupleurum chinense), Fructus Aurantii Immaturus (Citrus aurantium), Radix Paeoniae Alba (Paeonia lactiflora), and Radix Glycyrrhizae (Glycyrrhiza uralensis). It is traditionally used to harmonize the liver and spleen, move Qi, resolve constraint, and support digestive health. Modern research indicates that Si Ni San exhibits anti-inflammatory, anti-fibrotic, analgesic[1], hepatoprotective[3], immunomodulatory[4], and antidepressant effects[2][5]. Its mechanisms include downregulation of pro-inflammatory cytokines such as TNF-α and IL-1β; modulation of brain-derived neurotrophic factor (BDNF) pathways; inhibition of NF-κB signaling; regulation of cytochrome P450 enzymatic activity; suppression of T lymphocyte adhesion and matrix metalloproteinase production. Key active compounds identified in the formula include sinensetin, hesperetin, liquiritigenin, naringenin, quercetin, albiflorin and paeoniflorin[5]. Clinically studied indications include depression[2][5], chronic pancreatitis[1], hepatitis/liver injury[3], contact dermatitis/immune-related disorders[4] and gastrointestinal dysfunctions.
