Si-Ni-Tang is a classical Traditional Chinese Medicine (TCM) herbal formula first documented in the ancient medical text *Shanghan Lun*. It is a combination product composed of three primary herbal ingredients: processed *Aconitum carmichaeli* (Fuzi), *Zingiber officinale* (Ganjiang), and *Glycyrrhiza uralensis* (Zhigancao). The formula is traditionally used to treat conditions characterized by 'depletion of Yang Qi' and hemodynamic collapse, such as septic shock, heart failure, and severe diarrhea. Mechanistically, the alkaloids in *Aconitum* (such as higenamine and coryneine) provide cardiotonic, positive inotropic, and vasopressor effects. Simultaneously, the active components in *Zingiber* and *Glycyrrhiza* (such as gingerols and glycyrrhizin) exert potent anti-inflammatory effects by inhibiting the production of pro-inflammatory cytokines like TNF-α and IL-6 and suppressing lipopolysaccharide-induced macrophage activation. Si-Ni-Tang is currently being investigated in clinical trials as an adjunctive therapy to standard care for patients with severe sepsis and septic shock to improve hemodynamic stability and modulate immune function.
