Siagoside is the inner ester form of the monosialoganglioside GM1, a glycosphingolipid naturally occurring in high concentrations within the neuronal membranes of the central nervous system. Developed primarily by Fidia Farmaceutici, siagoside was investigated for its neuroprotective and neurorestorative properties in conditions such as acute spinal cord injury, ischemic stroke, and Parkinson's disease. Its mechanism of action involves the stabilization of cell membranes, modulation of ion pump activity (such as Na+/K+-ATPase), and the potentiation of endogenous neurotrophic factors like Nerve Growth Factor (NGF). Despite showing efficacy in some clinical trials, notably in improving motor recovery after spinal cord injury, its clinical use was hampered by concerns regarding an association with Guillain-Barré syndrome and inconsistent results in larger trials, leading to its discontinuation in many markets.
