Overview

Sihuang powder (四黄散) is a traditional Chinese medicine (TCM) topical formulation composed of four 'yellow' herbs, typically including *Phellodendron amurense* (Huang Bai), *Rheum palmatum* (Da Huang), *Scutellaria baicalensis* (Huang Qin), and *Coptis chinensis* (Huang Lian) or *Gardenia jasminoides* (Zhi Zi). It is widely utilized in clinical practice in China for the external treatment of inflammatory conditions, most notably infusion-induced phlebitis, as well as cellulitis and localized swelling. The formula is characterized by its 'heat-clearing,' 'detoxifying,' and 'blood-cooling' properties in TCM theory. Pharmacologically, its active constituents—such as berberine, baicalin, and emodin—exert anti-inflammatory effects by inhibiting pro-inflammatory cytokines and mediators. In the context of infusion-induced phlebitis, it is often applied as a wet compress or paste, sometimes combined with microwave irradiation to enhance therapeutic outcomes.
