This combination therapy involves the co-administration of oral sildenafil and topically delivered sodium nitroprussiate (SNP) via iontophoresis. Sildenafil is a selective inhibitor of phosphodiesterase type 5 (PDE5), which increases cyclic guanosine monophosphate (cGMP) levels by preventing its degradation, thereby promoting smooth muscle relaxation and vasodilation. Sodium nitroprussiate acts as a nitric oxide (NO) donor, which directly stimulates soluble guanylate cyclase to produce cGMP, further enhancing the vasodilatory effect. The combination is being investigated by the University Hospital, Grenoble, for its synergistic potential to improve cutaneous microcirculation. It is primarily studied as a therapeutic strategy for microvascular complications associated with Raynaud's phenomenon and systemic sclerosis, such as digital ulcers.
