Sildopin (SNG1004) is a fixed-dose combination (FDC) pharmaceutical product developed by Shinpoong Pharmaceutical, containing the calcium channel blocker amlodipine besylate and the phosphodiesterase type 5 (PDE5) inhibitor sildenafil citrate. It is indicated for the treatment of patients who have both hypertension and erectile dysfunction, a common clinical scenario as both conditions share underlying vascular pathophysiology. Amlodipine exerts its antihypertensive effect by inhibiting the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle, resulting in peripheral vasodilation and reduced blood pressure. Sildenafil treats erectile dysfunction by inhibiting PDE5, the enzyme responsible for the degradation of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum; this inhibition leads to increased cGMP levels, smooth muscle relaxation, and enhanced blood flow to the penis during sexual stimulation. The combination therapy is designed to improve patient compliance by reducing the pill burden for individuals managing these comorbid conditions.
