Silodosin + tadalafil is a fixed-dose combination of two small molecule drugs used primarily for the treatment of lower urinary tract symptoms (LUTS) associated with benign prostatic hyperplasia (BPH), particularly in men who may also have erectile dysfunction. Silodosin is a highly selective alpha-1A-adrenoceptor antagonist that relaxes smooth muscle in the prostate and bladder neck, improving urine flow and reducing BPH symptoms. Tadalafil is a phosphodiesterase type 5 (PDE5) inhibitor that increases cyclic guanosine monophosphate (cGMP) levels, leading to smooth muscle relaxation in the lower urinary tract and corpus cavernosum, thereby improving both LUTS and erectile function. Clinical studies demonstrate that this combination provides greater improvement in urinary flow rate, symptom scores, and erectile function than either drug alone[1][3][9]. The combination has also been studied as medical expulsive therapy for distal ureteric stones[4].
