Siltuximab is a chimeric (human-mouse) monoclonal immunoglobulin G1-kappa antibody that targets interleukin-6 (IL-6), a pro-inflammatory cytokine involved in immune regulation and inflammation. By binding directly to IL-6, siltuximab prevents it from interacting with both soluble and membrane-bound IL-6 receptors, thereby inhibiting downstream signaling pathways such as JAK/STAT. This blockade reduces the pathological immune activation seen in diseases characterized by excessive IL-6 production, notably idiopathic multicentric Castleman's disease (iMCD). Siltuximab is approved for the treatment of adult patients with multicentric Castleman's disease who are HIV-negative and HHV8-negative. It is administered as an intravenous infusion every three weeks[1][5][6][8].
