Silver nanoparticles reinforced double antibiotic paste (AgNP-DAP) is an experimental dental intracanal medicament developed by the All India Institute of Medical Sciences for the treatment of symptomatic apical periodontitis. The formulation combines a double antibiotic paste (DAP)—typically comprising ciprofloxacin and metronidazole—with silver nanoparticles to enhance antimicrobial efficacy against polymicrobial root canal infections and resistant biofilms. The silver nanoparticles provide a high surface-area-to-volume ratio, allowing for better penetration into dentinal tubules and disruption of bacterial cell membranes, while the antibiotics target bacterial DNA synthesis and replication. The paste is intended to reduce inflammatory biomarkers (such as IL-1β and MMP-9), alleviate post-endodontic pain, and promote periapical healing.
