Overview
Cleanance Comedomed Peeling is a topical dermo-cosmetic combination product developed by Avène (Pierre Fabre) for the treatment of mild to moderate acne vulgaris. The formulation contains 7% *Silybum marianum* fruit extract (marketed as Comedoclastin®), 0.1% retinaldehyde, and 6% glycolic acid. The active ingredients work synergistically to target multiple pathways in acne pathogenesis: Comedoclastin regulates keratinocyte differentiation and modulates sebaceous stem cell homeostasis to prevent microcomedone formation; retinaldehyde, a precursor to retinoic acid, promotes epidermal renewal and exhibits antimicrobial activity against *Cutibacterium acnes*; and glycolic acid, an alpha-hydroxy acid (AHA), provides keratolytic action to exfoliate the skin and improve texture. Clinical pilot studies have demonstrated significant reductions in total lesion count, comedones, and inflammatory lesions within 30 days of treatment.
