SIM0032 is an investigational small molecule inhibitor of aquaporin-4 (AQP4) currently under development by Simcere Pharmaceutical and its subsidiary, Shandong Simcere Medgenn. AQP4 is the most abundant water channel protein in the mammalian central nervous system, primarily localized on the foot processes of astrocytes where it maintains water homeostasis. In neuromyelitis optica spectrum disorder (NMOSD), pathogenic autoantibodies (AQP4-IgG) target this protein, leading to complement-mediated astrocyte damage and neuroinflammation. SIM0032 is designed to selectively inhibit AQP4, thereby potentially preventing the binding of autoantibodies and mitigating the inflammatory cascade. Beyond NMOSD, AQP4 inhibition is also being explored for its potential to reduce cerebral edema associated with conditions such as ischemic stroke and traumatic brain injury.
