SIM0237 + BCG is an investigational combination therapy currently in phase 1 clinical trials for non-muscle-invasive bladder cancer (NMIBC). SIM0237 is a bi-functional fusion protein developed by Simcere Zaiming, consisting of an anti-PD-L1 monoclonal antibody fused with a potency-reduced IL-15/IL-15Rα sushi domain. This design allows SIM0237 to block the PD-1/PD-L1 immunosuppressive pathway and simultaneously activate the immune system via its IL-15 component, promoting expansion and activation of NK cells and CD8+ T cells. The attenuation of IL-15 increases the maximum tolerated dose (MTD) while reducing risks such as overstimulation or anergy. When combined with BCG (bacillus Calmette-Guerin), which is a standard intravesical immunotherapy for NMIBC, this regimen aims to synergistically relieve tumor-induced immunosuppression and boost antitumor immune responses. The combination is being evaluated for safety, tolerability, pharmacokinetics, and preliminary efficacy in patients with NMIBC who may have relapsed or are refractory after prior therapies[1][4][5][8].
