Overview

Simeprevir (TMC435) is an oral hepatitis C virus (HCV) NS3/4A protease inhibitor developed for the treatment of chronic HCV infection. Efavirenz is a non-nucleoside reverse transcriptase inhibitor (NNRTI) used in combination with other antiretroviral agents to treat human immunodeficiency virus (HIV) infection. The combination of simeprevir and efavirenz has been studied primarily to assess pharmacokinetic interactions when co-administered, as both drugs are metabolized by the cytochrome P450 system and may affect each other's plasma concentrations[2][4]. Simeprevir inhibits viral replication by blocking the activity of the HCV NS3/4A protease, essential for viral polyprotein processing. Efavirenz inhibits HIV-1 reverse transcriptase, preventing viral RNA from being transcribed into DNA and thus halting HIV replication[8][1].
