Simfibrate is a small molecule drug classified as a fibrate and a derivative of clofibric acid. It acts as an antilipidemic agent, primarily used for the treatment of hyperlipidemia, including conditions such as arteriosclerosis, cerebral and coronary atherosclerosis, hypertension, and diabetes-associated lipid disorders. Chemically, it is a double ester of clofibric acid with 1,3-propanediol; in the body it is hydrolyzed to release two molecules of clofibric acid (the active lipid-lowering agent) and one molecule of 1,3-propanediol. The primary mechanism involves activation of peroxisome proliferator–activated receptor alpha (PPAR-alpha), which regulates genes involved in lipid metabolism—leading to increased lipolysis and elimination of triglyceride-rich particles from plasma. This results in lowered serum cholesterol and triglycerides[1][3][4][6][7].
