Simiao Pill + Mianjian Granule is a traditional Chinese medicine (TCM) formulation currently under clinical investigation for the treatment of hyperuricemia, particularly in patients presenting with damp-heat syndrome. The formulation is a granule-based preparation of the classical Simiao Pill (Simiao-Wan), which is composed of four primary herbal ingredients: *Phellodendron amurense* (Cortex Phellodendri Chinensis), *Atractylodes lancea* (Rhizoma Atractylodis), *Coix lacryma-jobi* (Semen Coicis), and *Cyathula officinalis* (Achyranthes bidentata). Its therapeutic mechanism involves the inhibition of xanthine oxidase (XOD) and xanthine dehydrogenase (XDH) to lower uric acid production, the modulation of renal transporters such as URAT1 and OAT1 to facilitate uric acid excretion, and the suppression of the NLRP3 inflammasome pathway to mitigate inflammatory responses. The drug is being evaluated in a Phase 0 randomized, placebo-controlled trial sponsored by Southern Medical University.
