Simpact is a fixed-dose combination antiretroviral therapy (ART) indicated for the treatment of human immunodeficiency virus type 1 (HIV-1) infection. Developed and manufactured by Aspen Pharmacare, it contains three active pharmaceutical ingredients: dolutegravir, lamivudine, and tenofovir disoproxil fumarate. Dolutegravir is a second-generation integrase strand transfer inhibitor (INSTI) that prevents the viral DNA from integrating into the host cell genome. Lamivudine and tenofovir disoproxil fumarate are nucleoside and nucleotide reverse transcriptase inhibitors (NRTIs), respectively, which act as chain terminators to block the conversion of viral RNA into DNA. This specific combination, commonly known as TLD, is recognized by global health organizations as a preferred first-line regimen due to its high potency, high genetic barrier to resistance, and improved tolerability compared to older efavirenz-based regimens.
