Simvastatin gel is an investigational local formulation of the HMG-CoA reductase inhibitor simvastatin, being developed by researchers at Ain Shams University for dental applications. While simvastatin is traditionally used systemically for hypercholesterolemia, this local application leverages its pleiotropic effects on bone metabolism. It promotes bone formation by increasing the expression of bone morphogenetic protein-2 (BMP-2) in osteoblasts and inhibits bone resorption by reducing osteoclast activity. The formulation is typically a 1.2% gel or a solid lipid nanoparticle carrier applied directly into peri-implant gaps or alveolar defects to enhance guided bone regeneration (GBR) and ridge preservation.
