Overview

Sinapultide is a synthetic 21-amino-acid peptide (sequence KLLLLKLLLLKLLLLKLLLLK, also known as KL4) designed to mimic the function of human pulmonary surfactant protein B (SP-B), which is critical for reducing surface tension in the lungs and stabilizing alveoli during respiration. It is formulated with phospholipids (DPPC, POPG) and palmitic acid to create the drug lucinactant. Sinapultide was originally developed at The Scripps Research Institute and later licensed to Windtree Therapeutics. Its primary indication has been for the prevention and treatment of respiratory distress syndrome (RDS) in premature infants, where it compensates for surfactant deficiency by restoring lung surface activity. Mechanistically, sinapultide acts by inhibiting neutrophil transmigration through endothelium, thereby reducing inflammation and lung permeability in models of lung injury[2][3][5][6].
