A single-target CLDN18.2 antibody-drug conjugate (ADC) is a targeted therapeutic designed to treat cancers overexpressing the tight junction protein Claudin 18.2, such as gastric, pancreatic, and esophageal adenocarcinomas. The molecule consists of a monoclonal antibody specific for CLDN18.2 linked to a potent cytotoxic payload. Upon binding to the target on the cell surface, the ADC is internalized via endocytosis, followed by the release of the payload (e.g., monomethyl auristatin E or a topoisomerase I inhibitor) within the lysosomal compartment. This mechanism allows for the concentrated delivery of chemotherapy to malignant cells while sparing healthy tissues with low or no CLDN18.2 expression. Several single-target CLDN18.2 ADCs are currently in clinical development, showing promising efficacy in patients with advanced gastrointestinal tumors.
