Overview

Sinistrin is a naturally occurring polyfructosan, a polymer of fructose, used as a diagnostic agent to measure glomerular filtration rate (GFR). Chemically similar to inulin but with improved water solubility, it serves as an ideal exogenous marker because it is freely filtered by the renal glomeruli and undergoes no tubular secretion, reabsorption, or metabolism. It is typically administered as an intravenous bolus to provide an accurate measurement of GFR (mGFR), particularly in clinical scenarios where creatinine-based estimation equations (eGFR) are known to be inaccurate, such as in patients with near-normal kidney function or in potential live kidney donors.
