Overview

Sintilimab + albumin-bound paclitaxel + gemcitabine is an investigational chemoimmunotherapy combination regimen being evaluated for the treatment of advanced malignancies. The regimen consists of **sintilimab** (Tyvyt), a fully human IgG4 monoclonal antibody that targets the Programmed Death-1 (PD-1) receptor to inhibit immune checkpoint signaling and restore T-cell antitumor activity; **albumin-bound paclitaxel** (nab-paclitaxel; Abraxane), a nanoparticle formulation of the taxane paclitaxel that disrupts microtubule dynamics to inhibit cell division; and **gemcitabine**, a nucleoside analog that interferes with DNA synthesis. This combination, often referred to as sintilimab plus the AG backbone, is primarily being studied as a first-line therapy for unresectable locally advanced or metastatic biliary tract cancers (BTC), including intrahepatic cholangiocarcinoma, extrahepatic cholangiocarcinoma, and gallbladder cancer. The strategy leverages the synergistic potential of combining cytotoxic chemotherapy with PD-1 blockade to improve objective response rates and survival outcomes in patients with biliary tract malignancies.
