This combination therapy consists of sintilimab, a PD-1 inhibitor, and tafolecimab, a PCSK9 inhibitor, being evaluated as a neoadjuvant treatment for rectal cancer. Sintilimab is a monoclonal antibody that blocks the PD-1 receptor on T-cells, preventing immune evasion by tumor cells. Tafolecimab is a monoclonal antibody targeting PCSK9; in the context of oncology, PCSK9 inhibition is hypothesized to increase the cell-surface expression of MHC-I and promote the infiltration of cytotoxic T-cells into the tumor microenvironment. This combination, initiated by Guangdong Provincial People's Hospital, specifically targets patients with mismatch repair-proficient (pMMR) or microsatellite stable (MSS) locally advanced rectal cancer, a population that typically exhibits poor responses to standard immune checkpoint inhibitor monotherapy.
