Sipuleucel-T (Provenge) is an autologous cellular immunotherapy designed to stimulate an immune response against prostate cancer. It consists of a patient's own peripheral blood mononuclear cells (PBMCs), primarily antigen-presenting cells (APCs), which are harvested via leukapheresis and incubated ex vivo with a recombinant fusion protein, PA2024. This fusion protein contains prostatic acid phosphatase (PAP), an antigen expressed in most prostate cancers, linked to granulocyte-macrophage colony-stimulating factor (GM-CSF), an immune cell activator. Once infused back into the patient, these activated APCs present the PAP antigen to T cells, inducing a targeted cytotoxic T-cell response against PAP-expressing prostate cancer cells. It was the first FDA-approved autologous cellular immunotherapy (2010) for the treatment of asymptomatic or minimally symptomatic metastatic castration-resistant prostate cancer (mCRPC). City of Hope Medical Center is currently evaluating sipuleucel-T in combination with the radioligand therapy 177Lu-PSMA-617 to assess potential synergistic effects in patients with mCRPC.
