This experimental cellular immunotherapy, developed by the National Cancer Institute (NCI), combines the mTOR inhibitor sirolimus (rapamycin) with sirolimus-generated donor Th2 cells (T-Rapa) for the treatment of metastatic renal cell carcinoma. The therapeutic approach involves a low-intensity allogeneic hematopoietic stem cell transplant (HSCT) protocol using a conditioning regimen of pentostatin and cyclophosphamide. Donor Th2 cells are specifically cultured ex vivo in the presence of sirolimus to render them resistant to the drug's immunosuppressive effects. These T-Rapa cells are then administered as multiple donor lymphocyte infusions (DLI) post-transplant. The strategy aims to induce a potent graft-versus-tumor (GVT) effect while concurrent high-dose sirolimus administration serves to prevent graft-versus-host disease (GVHD) without inhibiting the infused, resistant Th2 cells.
