Overview

Sisomicin is a broad-spectrum aminoglycoside antibiotic isolated from the fermentation broth of *Micromonospora inositola*. It is structurally related to gentamicin and exhibits potent bactericidal activity against Gram-negative bacteria and some Gram-positive bacteria, including strains resistant to other aminoglycosides. Sisomicin acts by binding to the 30S ribosomal subunit of bacterial cells, causing misreading of mRNA codons, which leads to defective protein synthesis and ultimately cell death. It is used primarily for the treatment of serious infections such as septicemia, respiratory tract infections, urinary tract infections, skin and soft tissue infections—including those caused by *Pseudomonas aeruginosa*—and ophthalmic conditions like conjunctivitis and keratitis. Like other aminoglycosides, it carries risks of nephrotoxicity and ototoxicity.
